Digital Messaging Supports Adherence and Positive Patient Experience in People with Obesity on GLP-1 RA Therapy

Background / Aim

Maintaining adherence to obesity treatment, including pharmacotherapy, remains a major challenge in the management of obesity as a chronic disease. Many individuals discontinue medications within the first three months, often due to side effects, limited follow-up, or insufficient behavioral support. This study evaluated a messaging-based intervention delivered via nymble health’s platform, designed to enhance medication adherence, provide ongoing education, and support behavioral change throughout the weight management journey.

Methods

Aggregated usage and outcome data were analyzed from users enrolled in obesity programs or receiving pharmacotherapy. The platform delivered phase-specific guidance tailored to treatment stages (initiation, titration, maintenance, and re-engagement), addressing side effect management, lifestyle behaviors, motivation, and treatment expectations. Content followed current obesity guidelines and behavioral science principles, emphasizing obesity as a chronic disease and promoting non-stigmatizing patient-centred engagement. The messaging-based design enabled scalable, accessible support for patients across geographic and digital literacy barriers. Outcomes included adherence at three months, user engagement (two or more bidirectional interactions per month), and qualitative feedback on perceived support and motivation.

Results

Across multiple deployments, aggregated data included over 15,000 eligible patient interactions. At three months, 83% of active users remained adherent to pharmacotherapy, compared with reported rates of approximately 40–50% under standard care. Engagement was observed in 65% of users, who reported improved understanding of treatment expectations, increased confidence in managing side effects, and motivation to continue therapy.

The most common discussion themes were medication access, dosing and practical administration (18.3%), acute side effects and symptom management (15.5%), weight plateaus and maintenance, and exercise (14.4%), appetite, cravings and nutrition strategies (14.4%). Users reported feeling supported through proactive, timely guidance that reinforced adherence and behavioral change. On average, the intervention saved clinicians approximately 50 hours per month per 100 users by addressing common questions and providing structured follow-up.

Conclusion

Messaging-based adherence support via nymble health was associated with higher short-term adherence and engagement, while providing ongoing education and behavioral guidance that frames obesity as a chronic disease and reduces stigma. Its scalable design enables consistent, equitable support across diverse populations. These findings suggest digital interventions can support patients in a patient-centered, scalable, and evidence-based manner, addressing multiple aspects of obesity management, including adherence, lifestyle behaviors, motivation, and equity in access.
